New Patient Portal Announcement
On May 6, 2024, Swan Mountain Women’s and Family Clinic has transitioned to a new electronic medical records system. We remain an independent practice, but now will share electronic medical records with the CommonSpirit hospital network. For dates of service PRIOR to May 6, 2024, you will have access to the portal for BILLING and RECORD REVIEW ONLY, until June 20, 2024. After this date, if you have questions records prior medical records or billing, you may contact our office at (970) 423-8840 or by email at [email protected]. Please refer to the links below for instructions regarding our new patient portal system.
To view your medical records or pay your statement balance for dates of service prior to May 6, 2024, please click “HERE”
To message your provider, view your medical records, or pay your statement balance for dates of service on or after May 6, 2024 please click the “MY CHART” link to create a MyChart account.
