Anthem Common Spirit Letter
Dear Patient:
If you receive your health insurance through the Anthem Blue Cross Blue Shield Colorado network, we wish to inform you that in recent months, CommonSpirit Health has been working with Anthem in good faith to reach a fair and balanced agreement that puts patients’ needs first and ensures your continued in-network access to providers and services.
Despite CommonSpriit Health’s best efforts, Anthem has terminated the contract with CommonSpirit Health effective 11:59 p.m. on April 30, 2024.
What you need to know:
Swan Mountain Women’s and Family Clinic (SMWFC) remains in-network and contracted with most Anthem Blue Cross Blue Shield plans. If your plan was previously in-network with SMWFC, you will still be able to see your provider at SMWFC with “in-network” benefits.
For our pregnant patients expecting to deliver and patients currently scheduled for surgery-
- Staff at SMWFC will provide you with a Continuity of Care authorization request form. You may also Visit www.anthemcommonspirit.com for an online application. You will need to complete this form and fax it to 800-763-3162 or mail to the address located on the form. Our staff will be more than happy to assist you with completing this form and will fax this on your behalf.
- Patients that are currently in active treatment or have appointments or procedures scheduled may also qualify for Continuity of Care Benefits. Visit www.anthemcommonspirit.com for more information.
- SMWFC will refer patients to in-network facilities and providers for those that are not currently scheduled or considered under the Continuity of Care agreement. Please check with your plan before you schedule appointments or visit facilities for procedures to ensure the network status and what your out-of-pocket expenses will be.
- Our billing team is readily available to speak with you regarding your concerns. Your message will be returned within 24-48 hours based on the volume of calls that we may receive. Please review our website and the www.anthemcommonspirit.com site to keep updated and informed.
• You may call 877-213-7081 to speak with a member of the Common Spirit care team who can answer your questions. Representatives are available 9am-9pm MT weekdays and 9am-5pm MT on weekends.
• If you receive insurance coverage through your employer, you may wish to speak with your HR or benefits manager about alternate or secondary coverage options that will protect your in-network access to CommonSpirit hospitals, clinics, physicians and providers.
